Out-of-network insurance benefits

Understanding superbills and possible reimbursement

Insurance benefits can be confusing. Although Forward Focus Counseling does not bill insurance directly, some clients have out-of-network benefits that may reimburse them for a portion of the cost of therapy.

Coverage varies by insurance plan, so reimbursement cannot be guaranteed. Your insurance company can tell you whether your particular plan includes out-of-network mental health benefits and how those benefits work.

1. Pay for your session

You pay the $205 session fee directly to Forward Focus Counseling at the time of service.

2. Receive a superbill

A superbill is a detailed receipt for therapy services that includes information an insurance company may require when processing an out-of-network claim. Forward Focus Counseling can provide you with a superbill.

3. Submit it to your insurance

You submit the superbill to your insurance company according to its claim process. If your plan provides out-of-network coverage, your insurer may reimburse you directly based on your plan's deductible, allowed amount, and reimbursement rate.

Questions to ask your insurance company

If you would like to find out whether you have out-of-network benefits, call the member-services number on the back of your insurance card and ask:

  • Do I have out-of-network benefits for outpatient mental health services?
  • What is my out-of-network deductible, and how much of it have I met?
  • After I meet my deductible, what percentage or amount does my plan reimburse for out-of-network therapy?
  • Is reimbursement based on the therapist's full fee or on an insurance company's allowed amount?
  • How do I submit a superbill or out-of-network claim for reimbursement?
  • Is there a form, online portal, or other documentation I need to submit with the superbill?

Out-of-network reimbursement is determined by your insurance plan. Forward Focus Counseling can provide documentation such as a superbill, but cannot guarantee whether or how much your insurer will reimburse.

What is a superbill?

A superbill is not a bill that you owe. It is documentation showing the counseling services you received and the amount you paid.

You can provide the superbill to your insurance company when requesting possible out-of-network reimbursement. Your insurer then determines whether the service is covered and what amount, if any, will be reimbursed under your plan.

If you are unsure how to submit one, ask your insurance company where to submit an out-of-network mental health claim or superbill.
